Meningitis and Hepatitis Vaccination Information

To abide by Florida law and University of South Florida policy, students living in on-campus housing must comply with the following guidelines.

Students applying for University housing after January 4, 2008 for all terms must provide evidence of Meningitis vaccination. In addition, students must provide proof of Hepatitis B vaccination or decline Hepatitis B vaccination in writing. If you received the Meningitis vaccine somewhere other than USF Student Health Services, please obtain documentation and submit it to the Immunization Compliance Office of Student Health Services. In addition, if you previously submitted a "blue form" indicating you received the Hepatitis B vaccine, you will now need to show proof of this vaccination OR fill out a new Immunization Health History form declining the vaccine. Meeting these requirements will ensure that you avoid immunization compliance problems during room sign-up in February.

Please note that returning students should already have information regarding Section A on file and Section B will need to be completed to live on campus. Also be aware that students living on campus do NOT have the option to decline the meningitis vaccine but DO have the option of declining the Hepatitis B vaccine.
